Tesamorelin nasal spray is a lipophilic peptide that has been proven to be effective in removing fat from the abdominal area during clinical studies. Initially, the research chemical was used in the treatment of lipodystrophy in patients with HIV, and is also being studied for its ability to reduce body fat in individuals with obesity.

Lipodystrophy is the medical term used for a build up of fat found in specific areas of the hips, abdomen and pelvis. Fat loss

Tesamorelin works by stimulating the pituitary gland to release the Growth Hormone, which increases lipolysis. Lipolysis is the breakdown of triglycerides and lipids to induce fat loss.

Lipids and triglycerides are fats found in the body stored for energy. High levels of either can increase your risk of ailments, including heart disease.

Research has found that Tesamorelin nasal spray works by reducing visceral fat. In clinical trials, mice given Tesamorelin for weight loss lost 15% of their weight in 26 weeks [1].

Furthermore, another 2014 study found Tesamorelin reduced liver fat by 18%. 

Lypodystrophy

Tesamorelin is primarily used to treat HIV-associated lipodystrophy, which occurs due to both HIV infection and antiretroviral treatment therapy. Lipodystrophy causes excessive fat accumulation specifically in the abdomen and elsewhere. The physiologic mechanism is unknown; however, it is suspected that protease inhibitors play a substantial role in lipodystrophy aetiology [2].

The last resort was surgery however it proved typically futile and complicated. This lead to FDA authorising Tesamorelin for HIV-associated lipodystrophy in 2010. HIV and GH deficiency

Recent research links Highly Active Antiretroviral Therapy (HAART) to endocrine and metabolic issues, including Growth Hormone insufficiency. This is due to HIV altering the pituitary gland, roughly one-third of people being treated with HAART unfortunately suffer Growth Hormone deficiency [4]. This may explain why lipodystrophy is so common in HIV patients and why tesamorelin proves as an effective therapy. Tesamorelin is a safe and effective alternative to exogenous GH, especially for those who are HIV-positive.

Furthermore, ectopic fat accumulation in lipodystrophy is linked to inflammation, which is a CVD risk factor. Visceral, hepatic, and epicardial fat are all related to a higher risk of Cardiovascular disease. Tesamorelin nasal spray reduces inflammation and therefore reduces CVD risk by lowering ectopic fat deposition.

Nerve Damage

Injury, diabetes, or surgery can cause peripheral nerve damage; however, little can be done to rectify the condition because nerve cells are notoriously difficult to regenerate. Research suggests that growth hormone manipulation therapy may improve peripheral nerve damage repair and speed [6]. Tesamorelin is now the frontrunner for such an intervention, partly due to FDA approval.

Dementia

There is currently evidence that GHRH analogues, including Tesamorelin nasal spray, can improve cognition in patients in the early stages of dementia [7]. For example, one study found that using  the Tesamorelin peptide or other GHRH analogues for 20 weeks increased GABA levels in the brain while lowering MI levels. These discoveries provide new areas for clinical research to be focussed in the search of a cure or prophylactic.
